12. Sexual & Reproductive Health
Overview
Physical health, anatomical understanding, and clinical awareness are integral to relationship wellbeing. Addressing pain, physiological changes, fertility, and reproductive health through an evidence-based medical lens empowers individuals and couples to seek timely care.
1. Anatomical & Pelvic Health Basics
- Anatomical Literacy: Understanding male and female pelvic anatomy, vascular structures, and nerve pathways.
- Pelvic Floor Function: The pelvic floor muscles support pelvic organs, urinary control, and intimacy. Hypertonicity (excessive tension) can cause pain during intercourse (dyspareunia or vaginismus).
2. Sexual Health, Contraception & STI Prevention
- Routine STI Testing: Regular STI screening is standard preventive care.
- Barrier Protection: Condoms and dental dams provide essential protection against STIs.
- Contraceptive Counseling: Working with a healthcare provider to choose barrier, hormonal, LARC, or non-hormonal contraception suited to life goals.
3. Clinical Symptoms & Medical Evaluation
3.1 Female Reproductive & Sexual Health Concerns
- Painful Intimacy (Dyspareunia / Vaginismus): Pain is a clinical signal requiring evaluation by an OB/GYN or pelvic floor physical therapist.
- Menstrual & Hormonal Conditions: Endometriosis, PCOS, severe dysmenorrhea, or perimenopausal tissue changes (vaginal atrophy) benefit from clinical treatment (e.g., localized estrogen therapy, physical therapy).
- Intimacy During Menstruation: Medically safe for consenting adults with barrier protection if STI/pregnancy risk applies — see Intimacy during menstruation. Pregnancy-stage intimacy: Sexual activity during pregnancy.
3.2 Male Sexual & Reproductive Health Concerns
- Erectile Difficulties (ED): Can stem from vascular, hormonal (low testosterone), neurological, medication-induced, or psychological factors.
- Ejaculatory Concerns: Premature or delayed ejaculation should be evaluated medically to rule out underlying prostatitis, thyroid dysfunction, or anxiety.
